General Service Agreement for Pet Care
and Permission to Enter

I/We, the undersigned, do hereby grant Abundant Love for Pets and its authorized representatives to enter my/our home located at:

for the purpose of caring for my/our pets(s) and/or home while I/we are away. I/We will not hold Abundant Love for Pets or its authorized representatives liable in anyway for anything not due to their negligence.

I/We will not hold Abundant Love for Pets or its authorized representatives liable for any acts of God over which I/we have no control such as flooding, hurricanes, tornadoes, fire or any other natural disasters, including forced entry of my/our home or for the death of the pets named to be cared for, due to natural causes or an act of God.

I/We understand and agree to pay the rate of $______________ per visit, which is due as discussed with me/us during the initial interview on _________________________. I/We further understand this rate may increase and I/we will be informed accordingly of any changes.
